Community South OB/GYN's obstetricians and gynecologists are certified by the American Board of Obstetrics and Gynecology or are board-eligible. Although we each have our own patients, we are associates and one of us is always available when the others are not – for obstetrical delivery, office care, hospital care or telephone consultation. Being individuals, we do have minor variations in our evaluations and treatment, but we are all dedicated to the most effective, practical solution to your individual problem.
April Hoffman, D.O.
Dr. April Hoffman is board certified in osteopathic obstetrics and gynecology. She received her doctor of osteopathic medicine degree in 2001 from Michigan State University, College of Osteopathic Medicine, East Lansing. She then completed her residency in obstetrics and gynecology, as well as an osteopathic internship at Michigan State University and Ingham Regional Medical Center, Lansing, Michigan (2002-2006). During this time, Dr. Hoffman was appointed as a clinical instructor in the Department of Osteopathic Surgical Specialties.
In addition to being board certified in her specialty, Dr. Hoffman has certifications in advanced cardiac life support (ACLS), basic life support (BLS) and neonatal resuscitation. Dr. Hoffman is a member of the Americal College of Osteopathic Obstetricians and Gynecologists, the American College of Obstetrics and Gynecology, and the American Osteopathic Association.
In her free time, April enjoys travel, photography and spending time with family.
Neal Ipema, M.D.
Dr. Neal Ipema attended the University of Illinois at Urbana-Champaign where he received his bachelor of sScience in biology. He received his doctor of medicine degree from Loyola Strich School of Medicine and completed his residency program in obstetrics and gynecology at Saint Joseph Hospital in Chicago, Illinois in 2010. Before joining Community Physician Network, Dr. Ipema spent two years in private practice in Hinsdale, Illinois.
Dr. Ipema practices the full spectrum of obstetrics and gynecology. He performs a wide range of minimally-invasive gynecological procedures and has a special interest in high-risk pregnancy. One of his passions is to provide medical missionary service to foreign countries, such as the Dominican Republic.
Dr. Ipema is excited to explore all that Greenwood and Indianapolis have to offer including the cuisine, biking trails and museums.
James Perry, M.D.
Dr. James Perry received his medical degree from Ohio State University in 1990 and completed his residency training at Indiana University Medical Center in 1994. He has been board certified in obstetrics and gynecology by the American Board of Obstetrics and Gynecology since 1996 and remains a current member. He was formerly the OB medical director for St. Francis Hospital, Mooresville, Indiana.
Since 1998, Dr. Perry has worked on the south side of Indianapolis for various hospitals and practices, including St. Francis-Mooresville Hospital, Southside OB/GYN and Southwest Center for Women's Health. Active in the medical community, he has held positions as president of the Morgan County Indiana State Medical Association, district president of the Indiana State Medical Association, director of obstetrics and gynecology services for Deaconness Family Practice Residency, and associate physician of Mt. Auburn Obstetrics in Cincinnati, Ohio.
In his free time, Dr. Perry enjoys cycling, fishing, running, golfing and skiing.
Nicole Perry, M.D.
Dr. Nicole Perry received her medical degree from University of Illinois in 1989 and completed her residency training at Indiana University Medical Center in 1993. She has been board certified in obstetrics and gynecology by the American Board of Obstetrics and Gynecology since 1996 and remains a current member.
Since 1998, Dr. Perry has worked on the south side of Indianapolis for various hospitals and practices, including St. Francis-Mooresville Hospital, Southside OB/GYN and Southwest Center for Women's Health.
In her free time, Dr. Perry enjoys cycling, skiing and cooking.
David Szentes, M.D., FACOG
Dr. David Szentes received his medical degree from Indiana University School of Medicine in 1992. He then completed an internship in internal medicine at West Suburban Hospital, Oak Park, Illinois, followed by residency training in obstetrics and gynecology at Mercy Hospital and Medical Center in Chicago. He is a fellow of and board certified in obstetrics and gynecology by the American Board of Obstetrics and Gynecology.
Dr. Szentes was formerly the chief of obstetrics at Johnson Memorial Hospital in Franklin, Indiana, where he serves on the surgery care review committee. He is also an active staff member at Community Hospital South. He served as president of the Johnson County Medical Society for five years (2001-2006) as well as on the performance improvement committee at Johnson Memorial Hospital.
Trista Stormer, R.N., WHNP-BC
Trista Stormer grew up in Holland, Michigan and received her bachelor’s degree in nursing from Purdue University in 2006. She moved to Indianapolis after graduating, where she worked as an registered nurse in labor and delivery and pediatrics. Trista graduated from the University of Indianapolis in 2012 with a master's in nursing and has obtained her certification as a women’s health nurse practitioner. She is very passionate about women’s health and patient education.
